The House Republicans stated that SGR Reform must: Not increase the deficit; Involve the physician community and other stakeholders; Foster clinically meaningful not government determined care for patients; Encourage achievable improvements in quality, efficiency, and patient outcomes based on physician-endorsed measures; Be applicable to all specialties, practice arrangements, and geographic locations; Reward the value rather than the volume of services; Motivate all stakeholders to adopt reforms; and Strengthen Medicare for seniors.

It is critical that physicians have a period of stability and incentives to invest in the management tools that will help them move away from the current, inefficient, fee for service models. With a new Congress it remains to be seen how this will proceed. Because there is little time before then to develop consensus around the legislation and how to pay for it, the odds again favor some sort of further short term extension.

But this could be done in a way that facilitates a permanent or semi-permanent fix soon, perhaps in this Congress. If Congress, adopts a new budget and moves to implement it, it is quite possible SGR will be included as one of those items. That process, however, can take several months to unfold. Now that the SGR has been repealed, it is likely that policy conversation about physician payments will move on to other outstanding issues, such as the balance of payments between primary care and other services and continued delivery system reforms, including population-based payments, with no fee-for-service component. Until the details of the measures of quality and value and incentive payments become available, it is uncertain if physicians will consider them reasonable and fair.

At some point, the cumulative effect of the new payment updates will not keep up with physician costs, unless the volume and cost of services substantially decrease, which is the same underlying issue as with the old payment updates.
